Position Summary:

Serves as liaison between Health System and the community to coordinate care for patient and their social services needs
Manages and completes administrative tasks associated with assessing, planning, implementing and evaluating/documenting psychosocial factors which contribute to patients' illnesses and/or have an impact on continuity of care
Scope of services may include, but is not limited to, assessing discharge planning needs or follow ups, providing counselled education to patient and/or families, domestic violence, drug and alcohol abuse, suspected abuse, psychiatric services and guardianships per regulatory requirements

Roles & Responsibilities:
Facilitates documentation of psychosocial assessments, chemical dependence consults, intervention and coordination of care to identified populations
Provides logistical and clerical support to enable inpatient social workers to focus on core responsibilities (i.e. completing forms that are not clinical in nature)
Coordinates referral and transition to post-acute services
Facilitates placement to rehab, skilled nursing, or other post-acute facilities
Maintains relationships with all post-acute providers and updates where available beds are located
Delivers and documents important messages from Medicare Second Notice, PASSR, PAE, etc.
Assists in completing transfer packet forms for post-acute providers
Facilitates discharge process by identifying and communicating with post-acute care facilities and community resources for the patient/family and performing the necessary administrative tasks to prompt community placement and/or addressing psychosocial / behavioral follow up appointments
Applies knowledge base, theoretical concepts, specific functional skills, and essential social values used to effect change in human behavior, emotional responses, and social conditions
In collaboration with Care Management and Utilization Management, monitors and develops actionable reports on trends in KPIs for the Care Management Department
Provides information, resource identification, referral services, mediation services, advocacy services and education of individuals, groups, couples, and families
Complies with standards and requirements in medical/legal reporting situations
Complies with social work competencies by department and hospital standards

Education and Experience Requirements:
- MSW required or BSW acceptable with commitment to earn master's degree in social services within five (5) years of hire.
- LSW or LCSW eligibility preferred depending upon facility requirements
- At least two (2) years of experience in a hospital/multidisciplinary healthcare setting preferred.
