Population Health Coordinator
The Population Health Coordinator will work with the Director and Primary Care providers to address education, workflow and other barriers to the overall health and wellness of the patient populations. This includes chronic disease management, health screenings and EMMC's own population health initiatives. The Coordinator will identify patients who are not meeting specific quality measures and work with providers and practice staff to ensure the patient is compliant and quality measures are met and improves the overall care of the patients through sustained workflow improvement. The Coordinator works with other members of the Medical Group Quality Team and the practice staff to reach mutual goals for quality of care and patient safety.
MINIMUM EDUCATION REQUIRED: Associates Degree
MINIMUM EXPERIENCE REQUIRED: 5-years in Healthcare
Bachelor's degree and/or 5-years in healthcare physician office setting strongly
Work experience in healthcare office quality/performance improvement or primary care office setting.
