The Social Worker is responsible for discharge planning for patients with complex issues, resource management, and other interventions to enhance patient functioning, ensure timely and appropriate discharge, emotional well-being, and access to services. The Social Worker is a professional caregiver with the special expertise required to provide services and crisis intervention to patients and families. The Social Worker strives to provide patients and families with support that improves their ability to cope with hospitalization, illness and resultant psychosocial problems. The Social Worker develops and facilitates relationships within the Health System and the community in order to provide services to a wide variety of patient populations. Family Service and Support Team will provide coordination of support services for children and families who have experienced or who are at high risk of abuse/neglect. Candidate will possess excellent communication skills, be able to multi-task, manage crucial conversations and function as a team player.
