Job Description
The Certified Community Behavioral Health Clinic (CCBHC) Behavioral Health Navigator role is designed to improve access to behavioral health services in the defined CCBHC service area through the coordination of care across the spectrum of health services, including access to high-quality physical health (both acute and chronic) and behavioral health care, as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and recovery of the whole person. Northern Light Acadia Healthcare’s Behavioral Health Navigator supports individuals on waiting lists for Northern Light Acadia Healthcare and Hospital services through short-term offerings of support/referrals.
Responsibilities:
  • Work in collaboration with other local CCBHC partners in support of the SAMHSA CCBHC grant. Works in compliance with Northern Light Acadia Hospital’s Subrecipient Agreement with Community Health and Counseling Services to deliver the CCBHC services.
  • Participate in regular coordination meetings with other CCBHC Navigators to discuss programmatic needs, brainstorm resources and support the overall goals of the program.
  • Respond to referrals made with timely outreach, flexible meeting location/availability and the provision of support or referrals to meet identified needs. Work with clients generally involves 1-3 face to face meetings or phone calls, provision of education, support and/or referrals to formal services.
  • Participate in activities designed to identify gaps and improve system coordination including membership on the CCBHC advisory committee.
  • Assist in program evaluation and required data collection activities as developed in collaboration with Community Health and Counseling Services and University of New England.
  • Secure appropriate releases to share Protected Health Information for the care and coordination of resources.
  • Interface with multiple Acadia Hospital and Healthcare departments to provide education about the CCBHC Behavioral Health Navigator role and recruit referrals.
  • Other Duties As assigned.
Other Information:
  • Willing to obtain provisional MHRT/C within 3 months.
